Below is our release and waiver form. 

Please complete all sections of this Atlanta Youth Tennis Waiver of Claims and Assumption of the Risk form and Medical Release, have it signed by the participant/attendee, and if such participant/attendee is a minor, have it signed by a parent or guardian. This form, signed by a parent or guardian and participant, must be completed through this registration in order to participate.

Waiver of claims and assumption of the risk:

Players, participants, and any spectators or individuals that are participating in or attending Atlanta Youth Tennis LLC, Dottie Bridges Tennis Courts, The Overhead Foundation, United States Tennis Association (USTA), National Junior Tennis and Learning (NJTL) tournaments, programs, or events (collectively, the “Releasing Parties”) acknowledge the risks of injury and/or illness associated with tennis and public events, accept those risks voluntarily, and in consideration of their acceptance in or at the specified Atlanta Youth Tennis LLC, Dottie Bridges Tennis Courts, The Overhead Foundation, United States Tennis Association (USTA), National Junior Tennis and Learning (NJTL) tournament, program or event (the “Event”), assume all loss or damage, and hereby waive and release any claim or demand against the Atlanta Youth Tennis LLC, Dottie Bridges Tennis Courts, The Overhead Foundation, United States Tennis Association (USTA), National Junior Tennis and Learning (NJTL) their affiliated entities (including Sectional Associations, State Associations, Districts, and Subdivisions), Dottie Bridges Tennis Courts school grounds, and the officials , employees, volunteers, and other agents or representatives of each of those entities involved with the Event (collectively, the “Released Parties”) on account of any injury to person and/or property; illness; exposure to or infection with communicable syndromes or diseases such as COVID-19; and/or death arising out of or related to the Releasing Party’s participation in the Event, whether caused by negligence or otherwise, to the fullest extent permitted by law (collectively, the “Released Claims”). The Releasing Parties expressly acknowledge that participation and attendance at the Event is voluntary and could increase the risk of contracting COVID-19, and assume all risks, whether known or unknown, to the fullest extent permitted by law.

Medical Release:

I hereby consent to the rendering of emergency first aid and related emergency medical care, which at the time of injury or illness seem reasonably advisable. I further understand that, if applicable, I will be responsible for payment of any such emergency medical care. In consideration of the acceptance of my entry or attendance at the Event, I hereby agree to abide by all applicable rules and regulations and codes of the Atlanta Youth Tennis Program, Dottie Bridges Tennis Courts conduct codes for this event, and if participating in the event, hereby consent to be tested for drugs pursuant to the provisions thereof.
